New Members will receive a Member ID Card through the US Mail at their home address.

 

Existing Member Options:

  1. Call Customer Service at 1-800-507-3800 and make the request.
     

  2. Go to www.superiorvision.com and print the card.

   3.  Download and use the mobile app which shows the member ID.
