| Offered by Block Vision The vision plan provides the most benefit when using in-network providers. The plan will pay for an annual eye exam in full. Lenses are paid in full every 12 months. Frames are covered every 24 months. A $150 allowance for contacts can be substituted for lenses every plan year. Rates are: Employee Only $ 5.85 Employee + One $9.80 Employee and Family $14.40 |
