This plan provides one eye exam and either one pair of glasses, or contact lenses, every 12 months. There is a $10 co pay for your eye exam, but no co pay for glasses or contacts. There is a $100 allowance for frames. Lenses for glasses are paid in full. Contact exams and lenses have a $125 annual allowance. |
