| This plan provides one eye exam and either one pair of glasses, or contact lenses, every 12 months. There is a $10 co-pay for your eye exam and a $10 co-pay for glasses or contacts. There is a $125 allowance for frames. Standard lenses for glasses are paid in full. Contact exams and lenses have a $150 annual allowance. |
