| Offered through Block Vision This plan provides one eye exam and either one pair of glasses, or contact lenses, every 12 months. There is a $10 co-pay for your eye exam and a $25 co-pay for glasses or contacts. There is a $150 allowance for frames. Standard lenses for glasses are paid in full. Contact exams and lenses have a $175 annual allowance. |
